Lifted Fitness is owned and operated by me, Rachelle Cheatham, NASM certified Personal Trainer and Nutrition Coach.
All gym routines included with memberships are created by me, so every Lifted Fitness member has access to my professional training services, although they are not personalized or modified for any one person's specific needs.

"Small Group" Training Rates:
2 Clients = $30 per person ($60 per hour)
Small groups comprised of clients with similar health/fitness goals

Private training facility occupied only by clients in small group
General nutrition guidance individualized as needed
2 or 3 day/week training available
"Hybrid" Training
A customized program is created based on individual health/fitness goals and current fitness/ability level. The client is responsible for executing the program and recording progress on their own time. The program is progressive, so the client is required to attend one weekly in-person OR virtual 30 minute check-in session per week so that the program can be assessed, progressed, and modified accordingly.
All hybrid clients must complete 4-6 one-hour in-person sessions before customized program is created
4-6 sessions cover assessment, nutrition guidance, movement mechanics, safety education. Exact number of sessions determined by client's current knowledge, skill level, and confidence without foundational movements
$37.50 per session per in-person
4-6 one-hour in-person sessions (required for assessment, instruction)
$225/6 week program includes:
6 week customized training routine
mandatory once/week 30 minute in-person OR virtual check-in
weekly form assessments video video communication
weekly review and feedback on weights, sets, reps
updated instruction and adjustments for upcoming weeks
Clients must adhere to mandatory check-ins and communication or will face termination. This structure ensures consistent support, accountability, and safe progression throughout the program.

Personal Training Information and Policies
Commitment/Contract:
All personal training clients- in-person or hybrid clients are committed to a 18 week minimum contract with the trainer (3 routine "cycles")
Hybrid clients: the 18 week minimum does NOT include the first 4-6 in-person introductory sessions for hybrid clients. The 18 week contract begins when the first 6 weeks routine of the written program has been assigned/paid for (program weeks 1-18).
In-person clients: 18 week contract begins on the first day of training. Assessment sessions/consultations are not included in 18 week contract.
Nutrition clients (weekly classes): 4 session minimum required
By purchasing sessions, the client is making a commitment to his/her health. Clients should follow the program and instructions of the trainer to the best of their ability to maximize their results and better achieve their goals. Trainer will show the client how to work their muscles correctly and encourage him/her to go to his/her safe limit, but the client is the only one who can make sure he/she works out consistently, eats properly, gets plenty of sleep, and lives a healthy lifestyle.
Goals will be assessed and adjusted frequently .Client's ultimate results are determined by their commitment to the program and lifestyle change both inside and outside of the training facility.
Specifics: Trainer and client shall agree upon the program frequency, duration, location and time of personal training sessions at the rate and location agreed upon.
Client must sign waiver/release of liability, PAR-Q questionnaire, and health history questionnaire prior to any training services.
Client is responsible for informing trainer of any changes to their physical or mental health and/or medications as soon as they are made aware.
Length of Sessions: Sessions will last approximately 60 minutes, including the warm-up and cool-down portion. Trainer and client may agree to vary the length of sessions, adhering to agreed upon rate.
Punctuality: Client shall be dressed and ready to train at agreed upon time. Failure to be prepared to train may result in a shortened workout or a possible cancellation of the session if client is more than fifteen (15) minutes late. If a client anticipates running late, he/she should contact the trainer as soon as possible.
Client agrees to pay entirely for a particular time slot, not a particular number of minutes trained. (i.e. if time agreed upon is 12-1 pm and client arrives 10 minutes late, client will only receive services for the remainder of the time slot- 50 minutes- but will be responsible for the entire hour's payment)
Attire: client must wear comfortable workout attire including, but not limited to, t-shirts, shorts, tights, sweats, and/or tracksuits. Athletic shoes must be supportive and functional. No open toed shoes or sandals permitted.
Stopping exercises: Client may refuse or stop any exercise for any reason. It is the client's responsibility to notify the trainer of any discomfort or pain arising from or during exercise, as well as any and all other known limitations the client has so that trainer may accommodate the client and substitute another exercise to work that particular muscle group.
Payment:
All one-on-one in-person and hybrid client session payments must be made in advance of sessions, at a minimum of one week in advance. All personal training services are billed separately from all other services and are not included with gym membership.
Hourly rate applies for any in-person services, including initial consultation.
a. Payment is currently accepted via cash, Paypal, Cashapp, Credit/Debit card, and ACH. Credit/Debit and ACH will be charged processing fees.

Cancellation of Individual Sessions:
A 72 hour cancellation notice is required for rescheduling or canceling any and all sessions for both one-on-one in-person clients and hybrid clients. Separate policies apply to small group clients.
Refunds will not be given, but pre-paid sessions may be rescheduled at an agreed upon time between client and trainer. If the pre-paid session cannot be rescheduled within a reasonable amount of time, left to the discretion of trainer, the client will forfeit the fee for the aforementioned training session.
Any session cancellation with less than 72 hours notice will result in a complete forfeiture of the session with no option to reschedule. If the trainer must cancel a session, she will do so with at least 72 hours notice or the client will not be charged for the missed session.
If client wishes to cancel all sessions for any week (due to vacation, work travel, etc.) then client is required to give one week advanced notice in order to avoid being charged.
If client does not give one week's notice for the cancellation of an entire week's sessions, then client is responsible for half of the entire weekly fee.
This policy will also be applied if client cancels more than one session per any one calendar week for reasons other than illness or emergencies.
Note: the above policy is implemented so that if a client must cancel several sessions, the trainer will have adequate time to arrange her schedule in order to meet business goals.
Cancellations/Refunds:
Refunds will not be given for any reason.
If for any medical reason client is no longer able to complete pre-paid training sessions, documentation from medical provider must be provided and client will receive a partial refund for outstanding sessions.
Partial or full refunds for any other reason are left to the discretion of trainer.
If a client- either in-person or hybrid- wishes to cancel services completely for any reason, a 30 day notice is required. Client must continue with sessions for the remainder of the 30 days. If client does not wish to continue the sessions for the 30 days, client is still financially responsible for the remainder of the 30 days at the rate at which they have paid for the previous sessions. Ie. if 2x/week for previous sessions, then client is responsible for payment of 2x/week for the remainder of the 30 days.
Relocation: Should client be unwilling or unable to complete sessions at the agreed upon location(s), trainer may cancel any and all remaining sessions.
Hybrid clients:
All hybrid clients are required to complete 4–6 one‑hour in‑person sessions before a customized program is created.
These sessions cover assessment, nutrition guidance, movement mechanics, and safety education. The exact number of sessions (4–6) depends on the client’s current knowledge, skill level, and confidence with foundational movements.
A client will only transition to independent training once the trainer determines they can perform exercises safely and effectively on their own.
Because scheduling varies, completing the required sessions typically takes 3–6 weeks, depending on both client and trainer availability.
Payment Policy for In‑Person Sessions
In‑person sessions are billed the weekend before the scheduled week of training.
Payment is due each Sunday, based on the number of sessions planned for the upcoming week.
Examples:
If you have two sessions scheduled that week, $75 is due on Sunday.
If you have one session, $37.50 is due on Sunday.
This advance payment reserves your session times and ensures consistent scheduling
Hybrid Program Delivery & Payment Policy
Once the required in‑person sessions are complete, the client will receive customized six‑week program.
The program is delivered on the final day of the in‑person sessions, and payment for the full six‑week program is due in full at that time.
The $225 program fee includes:
A fully customized six‑week training routine
Mandatory weekly in‑person check‑ins
Weekly form assessments via video communication
Weekly review and feedback on weights, sets, and reps
Updated instructions and adjustments for the upcoming week
This structure ensures consistent support, accountability, and safe progression throughout the entire six‑week program.
Client must adhere to mandatory check-ins or he/she will forfeit agreement and face possible termination
Update as of 1/6/25: All hybrid programs will now be periodized at 6 week increments. Weekly price remains the same at $37.50 per week. Six week payment= $225 due every SIX weeks rather than $150 every 4 weeks. This change has been implemented so clients can spend more time within the training block working on progressive overload in order to maximize strength and muscle gains and receive optimal results.
Client is responsible for communicating with trainer very thoroughly - including any discomfort or any health changes- at each check-in so that trainer can adjust and/or modify program if needed.
Client is responsible for performing any and all exercises safely and properly, including properly using exercise equipment, in order to avoid any injury. Client is responsible for contacting trainer with any questions regarding proper safety before using equipment if unfamiliar.
Refunds will not be given to "hybrid" clients under any circumstances. Once the monthly fee has been paid and client has gained access to program, any monies exchanged will be final.
If "hybrid" client has been terminated or has chosen to no longer move forward with trainer, client will retain access to any previously purchased programs.
"Hybrid" client is responsible for completing customized program on their own time.
Programs are created to provide sufficient workouts over a six week period. Programs are then updated based on client progression. Subsequent programs are billed in six week increments.
The programs are rewritten/modified every six weeks. The six week time period is not flexible. The six week time table will remain the same, regardless of whether or not client has completed all sessions. (Example- started six week program on July 1st, but failed to execute week 3 due to life conflicts. The week will still "count" as week 3 and the following week will continue as week 4).
